LIST OF REQUIRED DOCUMENTS

Nursery 1 – must be at least three (3) years old by the month of the school opening.
Nursery 2 – must be at least four (4) years old by the month of the school opening.
Kindergarten (Grade 1 prerequisite) – must be at least five (5) years old by the month of the school opening.

Credentials:

• Three (3) pieces of recent 2x2 ID pictures (with white background)Original PSA Birth Certificate with two (2) photocopies.
• Report Card (for Kindergarten)

GRADES 1 TO 6

Must be at least six (6) years old by the month of the school opening (for Grade 1)
Credentials:

• Original PSA Birth Certificate with two (2) photocopies.
• Three (3) pieces of recent 2x2 ID pictures (with white background)
• Learner’s Reference Number (LRN)
• NJHS Application Form (duly accomplished)
• Recommendation Form (accomplished by the Guidance Counselor from the previous school)
• Copy of student’s vaccination card (if available)
• Certificate of Good Moral Character
• ECCD checklist for incoming Grade One (1) pupils and /or Form 138 (Report Card) with Principal’s Signature

GRADES 7 TO 10

Credentials:

• PSA Birth Certificate
• Latest Progress Report Card (Form 138)
• Three (3) pieces of recent 2x2 ID pictures (with white background)
• NJHS Application Form (duly accomplished)
• NJHS Recommendation Form (accomplished by the Guidance Counselor from the previous school)
• Copy of student’s vaccination card (if available)
• Certificate of Good Moral Character
• ESC Transfer-out Certificate with ESC School ID and ESC Students ID for incoming Grades 8-10 (if applicable) 

Requirements for government subsidy / ESC (for incoming Grade7 only)

• Accomplished ESC Application Form (download here)
• Accomplished ESC Grantee Enrollment Contract (download here)
• Latest report card (Form 138)
• Latest BIR Income Tax Return (ITR) or any proof of monthly income from both parents.
• Submit an Affidavit of Non-Filling of ITR if parent has no work.
• Submit two (2) pieces 2X2 latest ID picture with white background.

GRADES 11 & 12

Credentials:

• PSA Birth Certificate
• Latest Progress Report Card (Form 138)
• Three (3) pieces of recent 2x2 ID pictures (with white background)
• NJHS Application Form (duly accomplished)
• NJHS Recommendation Form (accomplished by the Guidance Counselor from the previous school)
• Copy of student’s vaccination card (if available)
• Certificate of Good Moral Character
• Original copy of NCAE result with 2 photocopies
• Certificate of Completion of Junior High School

Requirements for students with dual citizenship or recognized as a Filipino citizen:

• Photocopy of Philippines Passport (biopage and latest note pages)
• Photocopy of latest Foreign Passport (biopage and latest note pages)
• Certificate of Recognition as a Filipino Citizen from the Bureau of Immigration
• Report of birth of child born abroad of Filipino parent
• Requirements for students from schools abroad:
• Authenticated academic records from the Department of Foreign Affairs (Consulate General of the Republic of the Philippines)
• English translation of documents.

Requirements for foreign documents:

• Photocopy of passport (data and latest note pages)
• Alien Certificate of Recognition or ACR-1 Card (front and back)
• Documents for the issuance of Special Study Permit (SSP)
• Special Resident Retiree’s Visa Card (front and back), if applicable.
